How do you create a poll or survey like this one?
KC100->CB100N->CB250RS--------->DL650AL2->R1200RS

If you can see that box then you have the ability to create a poll. The poll questions will be set up before positing or by clicking Preview.

The settings are made so that only admins and mods can create a poll. Would you like to create a survey, then I can do that for you, just send me a pm.
